Living With a Long-Term Condition? You Do Not Have to Manage It Alone

Long-term conditions such as type 2 diabetes, high blood pressure, heart disease, asthma and arthritis are part of everyday life for many Australians. You can read more about chronic conditions on the Australian Government health website. Living with one can feel like a second job, but ongoing support can make it feel far more manageable.

Good chronic disease care is not a single appointment. It is an ongoing partnership. Regular reviews let us check how you are going, review your medicines, monitor key measures and adjust your plan as life changes. Each visit builds a clearer picture. A review might cover how you have been feeling, your recent test results, any side effects from medicines, and the goals that matter most to you, whether that is more energy, fewer flare-ups or simply feeling more in control.

Lifestyle medicine is a particular interest of mine. Movement, nutrition, sleep, stress and social connection all influence how you feel day to day. I like to start small, with changes you can realistically keep, such as a short daily walk or one extra serve of vegetables. Small steps tend to last, and I would much rather build habits with you than hand you a list of rules. Every person living with a long-term condition has different priorities, and I enjoy learning what matters most to you.

I can also help coordinate care with other professionals, such as a dietitian or other allied health providers, so that everyone is working towards the same goals. Where you are eligible, Medicare may help with some allied health visits, though fees can apply for certain services, so it is worth asking our team. A dietitian, for example, can help turn general advice into practical meals that suit your tastes and your budget.

Living with a long-term condition can be tiring, and it is common to feel worn down at times. If that sounds familiar, please mention it. Looking after your mental wellbeing is part of looking after your health.
